JOB OVERVIEW
As a Licensed Practical Nurse (LPN) at Utilize Health, you will play a pivotal role in coordinating patient care throughout their journey with our organization. This position entails conducting comprehensive care coordination visits, both in the community and in patients’ homes. You will be responsible for guiding patients through the healthcare system, delivering crucial patient and family education to foster self-management, mitigating complications, addressing social determinants of health, and advocating for optimal patient outcomes and satisfaction.
JOB RESPONSIBILITIES
- Member of the Utilize Health multidisciplinary care coordination team, serving as an advocate from patient enrollment through graduation.
- Provide care coordination, health promotion, care planning and transitional care activities.
- Drive improved outcomes by reducing hospital and emergency room admissions, closing care gaps, and actively coordinating care for individuals with neurological diagnoses.
- Coordinate care effectively, supporting patients and families to achieve Utilize Health program goals.
- Perform ongoing assessments of patients’ physical and behavioral health needs, implement interventions, provide patient education, and develop individualized care plans with measurable goals.
- Ensure comprehensive planning and coordination of patients’ medical, behavioral, and environmental needs.
- Demonstrate proficiency in identifying and utilizing community and care resources.
- Actively participate in patient identification and enrollment processes.
- Engage in care team huddles, clinical case reviews, triages, and seamless handoffs.
- Facilitate patient access to care by liaising between patients and various care providers, including primary care physicians and specialists.
- Contribute to the development and dissemination of patient education materials, including action plans.
- Assist in the seamless transition of patients from hospital, acute rehab, or other inpatient settings to home.
- Maintain timely and accurate documentation in the Electronic Health Record (EHR).
OTHER RESPONSIBILITIES:
- Maintain an active Tennessee LPN License.
- Attend all required in-services, continuing education sessions, and annual health requirements.
- Adhere to all OSHA and safety policies and procedures, reporting any unsafe practices or accidents promptly.
- Ensure compliance with HIPAA regulations regarding patient privacy and security.
- Fulfill any other duties as assigned.

QUALIFYING JOB KNOWLEDGE, SKILLS AND ABILITIES
- Graduated from an accredited LPN Program
- Compact or State of Tennessee Active LPN Licensure
- American Heart Association’s BLS certification for HealthCare Providers required upon hire
- Two years of nursing experience; preference for experience in neurology, disease management, case management, or home health
- Ability to provide in-home visits
- Passion for working with individuals facing neurological challenges
- Demonstrated ability to provide care coordination
- Demonstrated proficiency in care coordination and electronic medical record documentation
- Excellent oral and written communication skills
- Proficient in Microsoft Office Suite
